Who Is Rosemarie
Rosemarie is the matriarch the Daughters of Lilith gather to honor, raised upon the sigil-stone while the lesser born kneel and bleed at its foot. Hers is the covenant that binds the flock into one will, and from her throne she spends that will like coin—hexing the strong, shielding the faithful, and turning her own wounds onto the bodies of her kneeling kin. The other sisters are talons; Rosemarie is the hand and the mind. The grimoires name her the throned mother of the brood and warn that so long as she holds her sigil aloft, the daughters neither fear nor falter, and every blow meant for her finds a worshipper instead.
Run Rosemarie in Sixty Seconds
- She is the boss then three legendary actions chain hexes and let the flock attack out of turn.
- Bleed the faithful then Sacrificial Ward redirects her damage onto kneeling sisters—kill the flock to reach her.
- Sigil makes them fearless then her aura blocks charm and fear and buffs every sister within 30 ft.

Medium fiend, lawful evil
CR 11 · 7,200 XP · GUIDELINEMatriarch's Sigil. Allied fiends within 30 feet of Rosemarie can't be charmed or frightened and add 2 (1d4) to one attack roll or saving throw of their choice once on each of their turns.
Sacrificial Ward. When Rosemarie takes damage, she can redirect that damage to one allied fiend within 30 feet instead (no action required, once per turn).
Innate Casting. Rosemarie innately casts (spell save DC 17): at will—darkness, bane, command; 3/day each—hold person, blight; 1/day each—fear, spirit guardians. She needs no material components.
Actions
Multiattack. Rosemarie makes two Ritual Blade attacks and uses Withering Word.
Ritual Blade. Melee Weapon Attack: +8 to hit, reach 5 ft., one target. Hit: 13 (2d8+4) slashing damage plus 9 (2d8) necrotic damage.
Withering Word. Rosemarie targets one creature she can see within 60 ft. The target must make a DC 17 Constitution save, taking 18 (4d8) necrotic damage on a failure, or half as much on a success.
Bonus Actions & Reactions
Call the Faithful (Bonus Action). Rosemarie directs one allied fiend within 30 feet to immediately make one weapon attack as a reaction.
Legendary Actions (3/Round)
Ritual Blade. Rosemarie makes one Ritual Blade attack.
Withering Word (Costs 2 Actions). Rosemarie uses Withering Word.
Sigil Surge (Costs 2 Actions). Each allied fiend within 30 feet of Rosemarie can use its reaction to move up to half its speed or make one weapon attack.
Throned upon a sigil-carved stone above prostrate worshippers, she wields the brood's covenant as a weapon, spending her sisters' fervor and her enemies' breath alike.

How It Fights — by Tier
Same creature, scaled threat. The traits never change; only the numbers move.
As the throned matriarch, Rosemarie anchors the whole brood: hold person and Withering Word from safety, redirect her wounds onto kneeling sisters, and Sigil Surge the flock for extra attacks. A true solo boss.
Open with hold person on the enemy leader, then Withering Word the back line; her sigil makes the flock fearless and her ward keeps her standing. Run her as the fight's centerpiece.
Command from the sigil-stone: legendary actions chain hexes and let the whole flock strike out of turn, while Sacrificial Ward bleeds her sisters to keep her alive. Run her as the warband's boss.

How a bound creature acts
- A companion acts on its owner's initiative, immediately after the owner.
- It uses the Companion block above — roughly ×0.6 HP and damage, no recharge abilities, and at most one reaction trait.
- It moves freely; it takes its Action only while its owner is conscious.
- At 0 HP a companion is broken, not dead — restored on a long rest unless your table rules otherwise.
