Umbralisk
twin-axe reaver with a maw in its chest
Umbralisk — The Encounter
twin-axe reaver with a maw in its chest · a boss your table can run with almost no prep. SRD 5.1-compatible.
Who Is Umbralisk
Umbralisk is rank-and-file butchery in the Dreadblood Maulers' front rank, one of the dual-axe reavers that wade in where the swarm wants a wall broken. The grimoires of the Lesser Key speak of the hungering dead that grow second mouths in undeath, feeding on the slain to knit their own torn flesh, and Umbralisk is exactly that horror given two heavy blades. It fights shoulder to shoulder with its kin, trading wounds gladly, because every body it drops feeds the maw and seals its hurts. Where Umbralisk swings, the grass is left wet, and the warband simply steps over what it leaves.
Run Umbralisk in Sixty Seconds
- Open on the soft target then swing both jagged axes and snap the chest-maw.
- Kill-priority is the nearly-dead then dropping a foe heals Umbralisk with Devouring Maw.
- When bloodied then go greedy and finish the weakest to self-heal.
- It's a wall, not solo then keep it elbow to elbow with other reavers.
Umbralisk
Medium undead, chaotic evil
CR 6 · 2,300 XP · GUIDELINEDevouring Maw. The fanged mouth in Umbralisk's torso hungers. Whenever Umbralisk reduces a creature to 0 hit points with a melee attack, it regains 7 (2d6) hit points.
Undead Fortitude. If damage reduces Umbralisk to 0 hit points, it makes a Constitution save (DC 5 + the damage taken, radiant or a critical hit auto-fails). On a success, it drops to 1 hit point instead.
Actions
Multiattack. Umbralisk makes two axe attacks.
Jagged Axe. Melee Weapon Attack: +7 to hit, reach 5 ft., one target. Hit: 13 (2d8+4) slashing damage.
Bonus Actions & Reactions
Maw Snap (Bonus Action). Umbralisk bites a creature within 5 ft.: Hit 7 (1d8+3) piercing damage. It can use this only against a target it hit with an axe this turn.
A red-skinned reaver hewing with two jagged axes while the lamprey mouth in its chest gnashes for the fallen.

How It Fights — by Tier
Same creature, scaled threat. The traits never change; only the numbers move.
Send Umbralisk straight at the nearest soft target with both axes. When it drops a foe, the chest-maw heals it, so let it carve through the weakest first. Keep it shoulder to shoulder with other reavers.
Open by charging the front-line fighter and swinging twice, then snap the maw at the same target. When bloodied it gets greedy: aim it at whoever is closest to dropping so the maw heals it. Fights as part of the reaver wall, never alone.
Three ways to run it: an anvil that pins the party's tank, a feeder set loose on downed PCs, or a wall-breaker paired with two more reavers. Run it as a warband piece, never solo.

How a bound creature acts
- A companion acts on its owner's initiative, immediately after the owner.
- It uses the Companion block above — roughly ×0.6 HP and damage, no recharge abilities, and at most one reaction trait.
- It moves freely; it takes its Action only while its owner is conscious.
- At 0 HP a companion is broken, not dead — restored on a long rest unless your table rules otherwise.

Questions, Asked at the Table
What CR is Umbralisk?
Umbralisk is CR 6 at its native Tier 2 (AC 15, 120 HP). Tier 1 (~CR 2) and Tier 3 (~CR 11) blocks are included to match your table.
How does Umbralisk fight alongside the rest of the Dreadblood Maulers?
It is a front-rank reaver, not a captain. Umbralisk holds the wall with other dual-blade kin, breaks the enemy line, and feeds its chest-maw on the fallen. Run it as part of the warband, never alone.
Does the chest-maw let Umbralisk heal forever?
Only when it actually reduces a creature to 0 hit points with a melee hit. No kill, no heal, so deny it easy finishers and the maw stays hungry.
