Axe-Maiden of the Cities of the Plain · a boss your table can run with almost no prep. SRD 5.1-compatible.

Who Is Lysandra

Among the burning cities of the plain, where indulgence curdled into cruelty, Lysandra was raised up from the smoke as an executioner of broken oaths. The grimoires name her the Axe-Maiden: a devil sworn to no throne but to the slow ledger of sin, sent wherever a pact is betrayed or a covenant defiled. She does not bargain. She arrives at the appointed hour, names the debt aloud in a voice only the guilty hear, and answers it with the cleaving edge. Those who watched the cities fall say she walked the rubble afterward, harvesting the unrepentant one swing at a time, her hair-flame brightening with each. Scholars warn that to summon her is to invite an accounting of one's own soul, for she reads guilt as plainly as scripture and is not careful to spare the conjurer. Where she treads, ash follows.

Cleaving Frenzy. Signature. Whenever Lysandra reduces a creature to 0 hit points with a melee attack on her turn, she can immediately make one melee weapon attack as part of the same action. This can occur once per turn.

Magic Weapons. Lysandra's weapon attacks are magical.

Sinner's Resolve. Lysandra has advantage on saving throws against being frightened or charmed, and she cannot be turned.

Actions

Multiattack. Lysandra makes two attacks with her greataxe.

Greataxe. Melee Weapon Attack: +7 to hit, reach 5 ft., one target. Hit: 17 (2d12 + 4) slashing damage plus 7 (2d6) fire damage.

Raking Talons. Melee Weapon Attack: +7 to hit, reach 5 ft., one target. Hit: 11 (2d6 + 4) slashing damage, and the target must succeed on a DC 14 Strength saving throw or be knocked prone.

Bonus Actions & Reactions

Bloodhunter's Step (Bonus Action). Lysandra moves up to half her speed toward a creature she can see without provoking opportunity attacks.

Spite (Reaction). When a creature within 5 feet of Lysandra hits her with an attack, she makes one Raking Talons attack against that creature.

A lean she-devil with magenta hide, clawed digitigrade legs, and a crest of living flame for hair, Lysandra plants her feet wide and hefts a fire-veined greataxe taller than her torso. Her spiked breastplate is scarred from a hundred sackings.

Ember Pact (signature). While within 30 feet of Lysandra, the Lysandra has advantage on saving throws, and the first creature it hits each turn takes an extra 3 (1d6) fire damage.

Actions

Searing Claws. Melee Weapon Attack: +5 to hit, reach 5 ft., one target. Hit: 9 (2d4 + 4) slashing damage plus 3 (1d6) fire damage.

Reactions

Cover the Maiden. When Lysandra is hit by a ranged attack, the Lysandra can leap into the path; it becomes the target and takes the damage instead.

A knee-high imp of smoldering coal that scampers at the Axe-Maiden's heels, snapping at anyone who would strike her from afar.

Open: Bloodhunter's Step + Multiattack on the lowest-HP target, banking on Cleaving Frenzy to roll into a second victim. Use Spite to punish whoever stands toe-to-toe with her. Kill-priority is the healer; if clustered, Raking Talons to knock a key target prone, then chop. Run her as the elite blade beside a bigger flagship boss or a knot of minions, never alone.

Lead with the Cinderling forward, then Step + Multiattack to start a Cleaving Frenzy chain through a clustered front line. Hold Spite for the heaviest melee striker. She is a finisher, not a controller, so pair her with a lair or a true centerpiece boss that locks down the party while she harvests the wounded.

What CR is Lysandra?

Her home build is CR 6 (2,300 XP), tuned for a party of levels 5-10. The record also gives a CR-2-ish Tier 1 version (levels 1-4) and a CR-11-ish Tier 3 version (levels 11-15); the traits stay identical and only the numbers move.

Can Lysandra be a companion or ally?

She is built as an adversary, but a lawful-evil party that strikes a binding pact could field her as a deadly mercenary blade. More naturally, the included Cinderling, Bound is the companion option: a small fire-imp ally that guards a chosen creature and adds fire damage to its strikes.

Is Lysandra meant to be fought alone?

No. She is an elite add-on, not a solo boss. With no legendary actions, a four-PC party out-acts her quickly, so deploy her beside a flagship threat, a lair, or a screen of minions where her Cleaving Frenzy can find wounded targets to chain through.
