Who Is Crescentmorph

Where a sorcerer dies mid-transformation and no one closes the rite, the unfinished shape keeps growing. Folk of the drowned hill-country call it the Crescentmorph, the crowned eye that wears stolen faces, and they bury suicides at crossroads precisely so it cannot find a half-living thing to inhabit. It is born already hungry, already wrong: one great eye opens in its chest before any other organ, and it learns the world by swallowing it. Every soul it kills does not depart but buds outward as a small dreaming face, and these faces remember their old names, and whisper them, and weep. The horns are not weapons but antennae of sight, a crescent fan that lets nothing approach unseen. Hunters who have survived it speak of being watched from every angle at once, of a calm sleeping mouth on its shoulder murmuring a dead brother's pet name into the dark. It does not roar. It recognizes you.

Crown of Crescents. The fan of curving horns rising from the Crescentmorph's back grants it 360-degree sight: it has advantage on Wisdom (Perception) checks and on Dexterity saving throws against effects it can perceive, and cannot be surprised while conscious. Attackers gain no benefit from being unseen or behind it.

Stolen Face. When the Crescentmorph reduces a humanoid to 0 hit points, a new dreaming face buds from its flesh. Once per turn as a free action it may have a stolen face whisper at one creature within 30 ft. that can hear it; that creature must succeed on a DC 14 Wisdom save or have disadvantage on its next attack roll or ability check.

Actions

Multiattack. The Crescentmorph makes one Maw attack and two Rending Claw attacks.

Maw. Melee Weapon Attack: +7 to hit, reach 5 ft., one target. Hit: 14 (2d10 + 4) piercing damage. If the target is a creature, it must succeed on a DC 14 Constitution save or be grappled (escape DC 14); while grappled it is restrained and takes 7 (2d6) psychic damage at the start of each of its turns.

Rending Claw. Melee Weapon Attack: +7 to hit, reach 10 ft., one target. Hit: 11 (2d6 + 4) slashing damage.

Spiral Stare (Recharge 5-6). The great eye fixes a creature it can see within 60 ft. The target must succeed on a DC 14 Wisdom saving throw or be stunned until the end of the Crescentmorph's next turn as nightmare-light pours into its mind. A creature that fails by 5 or more instead takes 18 (4d8) psychic damage and is stunned.

Bonus Actions & Reactions

Bristling Recoil. Reaction When a creature the Crescentmorph can see hits it with a melee attack, the crescent horns snap inward and the attacker takes 7 (2d6) slashing damage.

Reactions

Whispered Name. Reaction When an ally within 30 ft. is hit, the Budded Face whispers a dead name; the attacker must succeed on a DC 13 Wisdom save or the ally gains resistance to that attack's damage.

A lesser bud broken free of the parent horror, a single dreaming face wreathed in a half-crown of crescent horns, loyal to whoever speaks its forgotten name aloud.

Open with Spiral Stare to stun the loudest spellcaster, then Maw-grapple them and drag them into the horns. The Crown of Crescents means flanking does nothing, so don't waste actions repositioning. Pair it with weak skirmishers it can shepherd while it pins one PC.

Lead with Spiral Stare on the controller, then Maw the nearest melee PC into a restrained grapple where they eat 2d6 psychic each turn. Kill-priority is anyone who can grant the party advantage, since Crown of Crescents already denies them surprise and angles. Run it as the lieutenant beside a bigger flagship horror, not alone, so the party can't simply out-action it.

Stare-stun the heaviest single-target threat, grapple the frontline anchor, and let Bristling Recoil punish melee swarms. With Stolen Face debuffing each round, focus the healer so the grapple psychic damage tips someone past their saves. Field it amid budded faces and a lair of crossroad-dead, paired with a larger boss; the Crescentmorph is the relentless watcher, not the solo centerpiece.

What CR is Crescentmorph?

Natively CR 6 (Tier 2, ~levels 5-10): AC 15, 119 HP, +7 to hit, save DC 14. The record also gives drop-in Tier 1 (~CR 2) and Tier 3 (~CR 11) columns with identical traits and only the numbers shifted, so you can field the same horror at any table.

Can Crescentmorph be a companion or ally?

The full horror is a monster, but a lesser shard of it, Wakeful Face, Budded (AC 13, 71 HP), can serve as an ally: it shares its 360-degree watchfulness so your group can't be surprised, lashes with a horn, and whispers a dead name to shield a struck ally. Bind it to whoever speaks its forgotten name aloud.

Is Crescentmorph a solo boss?

No. It has no legendary actions and only one reaction, so a four-player party will out-act it if it fights alone. Run it as a lieutenant or relentless watcher beside a bigger flagship threat, supported by budded faces and a crossroad-dead lair.
