Who Is Nihiloth, Emissary of the Abyss
Where mystics speak of the abyss they mean no pit and no place, but the appetite beneath all places, the dark that was before light learned its name. Nihiloth is what that hunger sends to parley. It was a priest once, in a temple long swallowed, who bargained his body for the privilege of carrying messages no living tongue could survive. The void took the bargain and most of the priest with it; what walks now is a gilded vessel, polished and crowned, kept beautiful so that mortals will listen before they understand. Its scepter cradles a brain that is not its own, the last witness it consumed, kept screaming so the emissary never forgets what speech costs. Negotiators who meet it report the same thing: a courtesy so perfect it terrifies, and the slow certainty that every word spoken is being measured for how cleanly it can be erased.
Run Nihiloth, Emissary of the Abyss in Sixty Seconds
- Aura first. Voidspeech Aura punishes anyone who starts a turn within 15 ft.; melee PCs eat psychic damage just for closing, so make them choose between the aura and disengaging.
- Open with Abyssal Devouring on the tightest cluster, then Multiattack the Sickle into a healer to lock out hit-point recovery for a round.
- Kill-priority: the back line. Nihiloth wants enemy casters and healers; it ignores tanks. Protect your support or it shreds them.
- Whisper of Unmaking is the trap. Hold the reaction for the party's biggest spell to gut its DC or attack roll, not the first cantrip.
- Three ways to run it: (1) lich-priest controller behind minions, (2) parley-turned-ambush with the reliquary screaming, (3) the brains beside a heavier melee boss it refuses to leave.

CR 6 · 2,300 XP · GUIDELINEVoidspeech Aura. When a creature that can hear Nihiloth starts its turn within 15 feet of it, that creature must succeed on a DC 14 Wisdom save or take 5 (2d4) psychic damage as the emissary murmurs the names that things have before they are unmade.
Hollow Vessel. If Nihiloth is reduced to 0 hit points, it does not die at once. Instead it falls inert; at the start of its next turn it makes a DC 13 Constitution save, rising with 1 hit point on a success. It may attempt this once per day.
Actions
Multiattack. Nihiloth makes one Sickle of Severance attack and casts one spell from its repertoire, or it casts two spells if it does not attack.
Sickle of Severance. Melee Weapon Attack: +6 to hit, reach 5 ft., one target. Hit: 13 (2d8+4) slashing damage plus 9 (2d8) necrotic damage, and the target cannot regain hit points until the end of its next turn.
Abyssal Devouring (Recharge 5-6). Nihiloth opens a rent of starless void in a 30-foot cone. Each creature in the area makes a DC 14 Dexterity save, taking 36 (8d8) necrotic damage on a failure, or half as much on a success. A creature that fails is also pulled 10 feet toward the emissary.
Bonus Actions & Reactions
Whisper of Unmaking (Reaction). When a creature within 30 feet of Nihiloth that it can see casts a spell, the emissary speaks a counter-name. The caster must succeed on a DC 14 Constitution save or take 7 (2d6) psychic damage and have disadvantage on the spell's attack roll or save DC until the end of its turn.
A gaunt skeletal envoy crowned in a halo of golden flame-horns, bare-chested beneath ceremonial gilding, one hand gripping a curved sickle and the other a brain-topped scepter that screams without breath. Tattered violet shadow trails from its shoulders like the wings of something that gave up flight long ago.

Bound · GUIDELINEWitness Kept (signature). The reliquary cannot be silenced by nonmagical means. While its bearer can hear it, the bearer has advantage on saves against being charmed or frightened.
Actions
Lash of Names. Melee Weapon Attack: +5 to hit, reach 10 ft., one target. Hit: 10 (2d6+3) psychic damage, and the target must succeed on a DC 13 Wisdom save or be unable to speak until the end of its next turn.
Reactions
Recoiling Echo. When the bearer takes psychic damage, the reliquary screams; the triggering creature takes 5 (1d10) psychic damage.
A staff-mounted skull cradling an exposed, pulsing brain, kept alive and aware so it can never stop confessing the names of the dead.

How It Fights — by Tier
Same creature, scaled threat. The traits never change; only the numbers move.
Open with Voidspeech Aura forcing a save, then keep at range and chip with cantrip-tier necrotic bolts. Save Abyssal Devouring for a clustered party. Pair it with skeletal cultists so the emissary never has to melee.
Open with Abyssal Devouring into the tightest cluster, then Multiattack a caster with the Sickle to shut down healing. Hold Whisper of Unmaking for the enemy mage's first big spell. This is a controller add-on; pair it with a bigger melee threat to anchor the front while Nihiloth dismantles the back line.
Lead with the aura and Whisper of Unmaking to tax every spell, then alternate Abyssal Devouring and Sickle to deny healing on the party's healer. Lean on Hollow Vessel to bait an overcommit. Run it inside a lair with lesser undead minions and a heavier boss; it is the brains of the encounter, never the lone threat.

How a bound creature acts
- A companion acts on its owner's initiative, immediately after the owner.
- It uses the Companion block above — roughly ×0.6 HP and damage, no recharge abilities, and at most one reaction trait.
- It moves freely; it takes its Action only while its owner is conscious.
- At 0 HP a companion is broken, not dead — restored on a long rest unless your table rules otherwise.

What CR is Nihiloth, Emissary of the Abyss?
Its home tier is CR 6 (Tier 2, levels 5-10): AC 15, HP 123, +6 to hit, save DC 14. The table also gives a CR 2 build for levels 1-4 and a CR 11 build for levels 11-15; traits and actions are identical, only the numbers move.
Can Nihiloth be a companion or ally?
Not directly, but its scepter can. The Screaming Reliquary travels as a bound companion (AC 13, HP 74) that wards its bearer against fear and charm and lashes foes with psychic names. A patron who treats with the void rather than fighting it might be loaned the reliquary, never the emissary itself.
Is Nihiloth a solo boss?
No. It is a controller add-on with no legendary actions, so a four-PC party out-acts it on its own. Run it as the intelligence behind an encounter, paired with a heavier melee threat or a screen of lesser undead, and let it dismantle the back line while something else holds the front.
