Tentacle-faced thrall of the void between stars · a boss your table can run with almost no prep. SRD 5.1-compatible.

Who Is Quorilith

Recorded in the marginalia of drowned star-charts, the Quorilith is named among the thralls that descended from the lightless gulf beyond the fixed stars — kin to the deep-dwellers and the spawn of sleeping titans, not born of any earthly womb. Scholars who survived its passing describe a hunger that is not for flesh but for thought itself: it peels the surface from a mind the way a glutton strips a shell, and leaves the body walking and empty. Its tentacled visage marks it as a herald of the outer void, and the suckered staff it bears is said to be a lesser cousin grafted into wood and bound to obedience. Where one Quorilith is found, others wait below, building drowned colonies in caverns no sun has touched. It does not negotiate. It listens, and when it has heard enough of you, it reaches in.

Mind-Reaving Presence. Signature. The Quorilith perceives the surface thoughts of any creature within 60 ft. that can think. The first time on each of its turns it hits a creature it has read this way, that creature has disadvantage on its next saving throw before the start of the Quorilith's next turn — it strikes where the mind is already buckling.

Void-Touched Mind. The Quorilith has advantage on saving throws against being charmed or frightened, and magic can't put it to sleep.

Actions

Multiattack. The Quorilith makes two attacks: one with its Tendril Staff and one Tentacle Lash, or it casts Mind Blast (recharge 5–6) in place of both attacks.

Tendril Staff. Melee Weapon Attack: +6 to hit, reach 10 ft., one target. Hit: 13 (3d6 + 3) bludgeoning damage plus 7 (2d6) psychic damage.

Tentacle Lash. Melee Weapon Attack: +6 to hit, reach 5 ft., one creature. Hit: 9 (2d6 + 2) psychic damage, and the target must succeed on a DC 15 Wisdom saving throw or be grappled (escape DC 14) as facial tentacles seize its skull.

Mind Blast (Recharge 5–6). The Quorilith unleashes a cone of psychic force. Each creature in a 30-ft. cone must make a DC 15 Intelligence saving throw, taking 27 (6d8) psychic damage on a failed save and being stunned until the end of its next turn, or half damage and not stunned on a success.

Bonus Actions & Reactions

Reaving Recoil (Reaction). When a creature the Quorilith can see within 60 ft. casts a spell or moves out of its reach, the Quorilith lashes a tendril at it: the target must succeed on a DC 15 Wisdom saving throw or take 9 (2d8) psychic damage and have its speed halved until the end of its next turn.

A gaunt, robed humanoid whose face has unraveled into a writhing cluster of glistening tentacles, gripping a staff wound with living suckered tendrils. It leans forward as though already inside your skull.

Hollowed (signature). The Quorilith is immune to being charmed or frightened, and while within 60 ft. of the creature that bound it, it can't be reduced to fewer than 1 hit point by psychic damage.

Actions

Reaving Touch. Melee Weapon Attack: +5 to hit, reach 5 ft., one creature. Hit: 11 (2d8 + 2) psychic damage, and the target can't regain hit points until the start of its next turn.

Reactions

Take the Thought. When an ally within 30 ft. fails an Intelligence or Wisdom saving throw, the Quorilith may take 7 (2d6) psychic damage to grant that ally a reroll.

A husked thrall, its will scooped out and replaced, that shambles in service to whatever reached into it last.

Open by reading minds, then hit the softest save with the Tendril Staff so it strikes at disadvantage. Lead with Mind Blast the moment two or more PCs cluster — a stun at this tier can end the fight. Pair it with a single melee brute or a few thralls so it can hang back and pick targets.

Open with Reaving Recoil-baiting positioning: stand 35 ft. back so movers and casters eat the reaction. Drop Mind Blast on the tightest cluster, then Tentacle Lash to grapple the party's spellcaster and shut down their concentration. Best run as the brains behind a melee line or a pack of thralls — it punishes whoever ignores it.

Kill-priority targeting: read the highest-INT PC and break their concentration first with the grapple. Recharge Mind Blast aggressively into stacked formations for the stun. At this tier it cannot solo a party — anchor it inside a drowned lair with thralls and guardians, or pair it with a bigger threat so it plays the controller while the muscle holds the line.

What CR is Quorilith?

Natively CR 6 (Tier 2, ~levels 5–10): AC 15, 127 HP, +6 to hit, save DC 15. The stat block scales down to ~CR 2 for Tier 1 and up to ~CR 11 for Tier 3 by swapping only the numbers in the tier table; the traits and actions stay identical.

Can Quorilith be a companion or ally?

Not directly — it's a coldly self-interested void-thrall. But it brings a bound ally: the Vessel, a hollowed thrall (AC 13, 76 HP) that shields the Quorilith from psychic damage and can take a thought to grant a reroll. Use the Vessel as the controllable piece if a player wants a companion of this kind.

Is Quorilith solo-boss material?

No. It's a controller add-on with one punishing reaction but no legendary actions, so a four-PC party will out-act it alone. Field it alongside a melee threat, thralls, or in a defensible lair where its Mind Blast and grapple can shine without it being dogpiled.
