Sneaklevenom — The Encounter

Bloated bile-spawn of the deceiver, weeping plague from the marrow · a boss your table can run with almost no prep. SRD 5.1-compatible.

Who Is Sneaklevenom

When the deceiver was bound beneath the roots and venom dripped eternal upon his brow, a single drop is said to have fallen wide and struck the marsh. There it pooled, soured, and stood up. Sneaklevenom is that runoff given hunger: a walking abscess of Loki's spite, swollen on every plague it has shrugged off. It does not march so much as ooze forward, and the ground behind it blackens. Healers who have faced it report that wounds taken in its presence simply refuse to close, weeping the same green that weeps from its hide. It hoards trophies in the form of skulls picked clean by its own pestilence, arranging them at its feet like a tithe. It feels little pain and less fear, for it has already rotted past the point where either matters. Where it dies, nothing grows for a season, and the water tastes of bile until the frost comes.

Weeping Pestilence. Signature. At the start of each of Sneaklevenom's turns, every creature within 10 feet of it must succeed on a DC 15 Constitution saving throw or take 7 (2d6) poison damage and be poisoned until the end of its next turn. A creature that fails by 5 or more is also infected: while infected it cannot regain hit points and takes 3 (1d6) poison damage at the start of each of its turns until it succeeds on a DC 15 Constitution save at the end of its turn (a creature can attempt the save once each turn).

Festering Bulk. Sneaklevenom has advantage on saving throws against being knocked prone, grappled, or moved against its will, and its bloated body smothers fire that touches it.

Actions

Multiattack. Sneaklevenom makes one Spiked Lance attack and one Scrap-Flail attack.

Spiked Lance. Melee Weapon Attack: +7 to hit, reach 10 ft., one target. Hit: 13 (2d8+4) piercing damage plus 4 (1d8) poison damage.

Scrap-Flail. Melee Weapon Attack: +7 to hit, reach 10 ft., one target. Hit: 11 (2d6+4) bludgeoning damage, and the target must succeed on a DC 15 Strength saving throw or be knocked prone.

Bile Spew (Recharge 5-6). Sneaklevenom retches a 15-foot cone of caustic bile. Each creature in the area must make a DC 15 Dexterity saving throw, taking 28 (8d6) poison damage on a failed save, or half as much on a success. A creature that fails is infected as described in Weeping Pestilence.

Bonus Actions & Reactions

Burst of Rot. Reaction. When Sneaklevenom takes slashing or piercing damage from a creature within 10 feet, it sprays infected ichor; the attacker must succeed on a DC 15 Constitution saving throw or take 5 (1d10) poison damage.

A grotesquely swollen brute of greenish, pustule-knotted flesh, its grinning maw split wide above a bolted collar, hauling spiked scrap-weapons on jointed arms as it wades through a litter of stripped skulls.

Carrion Devourer (signature). When Sneaklevenom reduces a creature to 0 hit points or hits a creature that is poisoned, it regains 5 (1d10) hit points.

Actions

Rending Bite. Melee Weapon Attack: +6 to hit, reach 5 ft., one target. Hit: 12 (2d8+3) piercing damage, and a poisoned target takes an extra 4 (1d8) poison damage.

Reactions

Swarm Cover. When an ally within 5 feet is hit by an attack, Sneaklevenom can crawl into the blow, reducing the damage by 5 (1d10) and taking that much itself.

A churning mound of fat carrion-grubs grown to the size of a hound, spawned from Sneaklevenom's seeping wounds and loyal to its rot.

Open by wading into the densest knot of heroes so Weeping Pestilence taxes as many as possible, then Scrap-Flail the front-liner prone. Kill-priority is any healer, since infected wounds won't close. Pair it with a single weak skirmisher; this thing is a wall, not a duelist.

Lead with Bile Spew the moment two or more PCs cluster, then Multiattack to anchor the line and spread infection. Make casters and healers the kill-priority; the no-healing infection is its whole point. Run it as the muscle beside a smarter spellcaster boss who herds the party into its reach.

Position it as a plague-warden in a fetid lair: open with Bile Spew, hold the choke point, and let Weeping Pestilence grind anyone who lingers. Burst of Rot punishes melee swarmers, so squishy strikers should kite. It is an add-on bruiser even here, so pair it with a bigger threat or fill the room with Maggotbrood minions.

What CR is Sneaklevenom?

Natively CR 6 (2,300 XP), a Tier 2 brute for parties of levels 5-10. Stat blocks for Tier 1 (~CR 2) and Tier 3 (~CR 11) are provided so you can scale it to your table.

Can Sneaklevenom be a companion or ally?

Not directly, it is a mindless agent of rot. But it travels with Maggotbrood, a bound carrion-grub swarm that a foul party (or a desperate plague-cult NPC) could field as an ally; its stats are included.

Why doesn't healing work near it?

Its Weeping Pestilence trait and Bile Spew can infect creatures, and an infected creature cannot regain hit points until it shakes the disease. That makes Sneaklevenom a brutal counter to healer-reliant parties, so warn players to spread out and cleanse the infection fast.
