Umbralisk — The Encounter

Horned brute of the pit, twin-axed and ever-hungry · a boss your table can run with almost no prep. SRD 5.1-compatible.

Who Is Umbralisk

The grimoires name the umbralisk among the foot-soldiers of the pit: a thing of appetite given limbs and horns. Where lesser demons are cunning, the umbralisk is simple hunger made flesh, and it was shaped, the scribes insist, from the gluttony of mortals who devoured their kin in famine. The second mouth that splits its torso is no wound but a true maw, and it feeds even as the creature fights, swallowing the blood and meat of whatever its axes open. Conjurers prize it for siege and ambush, for it tires neither in heat nor frost, and steel that bears no blessing splits upon its hide. Yet it cannot be commanded so much as aimed, for once the killing begins it pursues the dying with a frenzy no binding sigil fully restrains. Those who have survived its passing speak less of its blades than of the wet grinding of teeth where its breastbone ought to be.

Magic Resistance. The umbralisk has advantage on saving throws against spells and other magical effects.

Hunger of the Second Mouth. The umbralisk's torso splits into a fanged maw. Once per turn when it hits a creature it has already wounded this turn with both axes, that creature is grappled (escape DC 16) and drawn to the maw. While grappled this way it takes 7 (2d6) piercing damage at the start of each of the umbralisk's turns, and the umbralisk regains that many hit points.

Actions

Multiattack. The umbralisk makes two Twin-Axe attacks.

Twin-Axe. Melee Weapon Attack: +8 to hit, reach 5 ft., one target. Hit: 16 (2d10 + 5) slashing damage.

Horn Gore. Melee Weapon Attack: +8 to hit, reach 5 ft., one target. Hit: 13 (2d8 + 5) piercing damage, and if the target is a Large or smaller creature it must succeed on a DC 16 Strength saving throw or be knocked prone.

Bonus Actions & Reactions

Bloodlust (Reaction). When a creature the umbralisk can see drops to 0 hit points within 30 feet, the umbralisk can move up to half its speed and make one Twin-Axe attack.

A towering crimson brute crowned with sweeping curved horns, the Umbralisk grins with both its jagged jaw and the fanged mouth gaping in its chest. In each fist it whirls a double-headed axe, and spiked iron plates ride its shoulders and shin.

Feast-Frenzy (signature). While a creature within 10 feet of Umbralisk is below half its hit points, Umbralisk has advantage on melee attack rolls against that creature.

Reactions

Snapping Jaws. When a creature within 5 feet hits Umbralisk with a melee attack, Umbralisk bites it for 5 (1d6 + 2) piercing damage.

A lesser spawn of the same hungering brood, Umbralisk trails its larger kin to gorge on whatever it leaves behind.

Open by charging the squishiest caster, Horn Gore to drop them prone, then both axes to trigger Hunger of the Second Mouth and grapple them to the maw. Kill-priority is anything below half HP because Bloodlust lets it lunge at the dying. Pair it with a bigger threat or a pack of minions; it is a frontline brute, not the whole fight.

Run it as the muscle in a demon warband or lair: it wades to the front line, grapples a target into the second mouth, and self-heals while a commander or sorcerer works the back. Focus Bloodlust on whoever the party drops first. Pair it with a bigger threat; alone it cannot out-act a high-tier party.

What CR is Umbralisk?

Natively CR 6 (2,300 XP) — a Tier 2 frontline brute for levels 5-10. The stat record also gives scaled Tier 1 (levels 1-4) and Tier 3 (levels 11-15) versions; only the numbers change, the traits and actions stay the same.

Can Umbralisk be a companion or ally?

Not the umbralisk itself — it is a chaotic-evil demon that cannot truly be commanded, only aimed. For a bound ally, use Ghaunt, a lesser hungering spawn (AC 14, 76 HP) that feeds on whatever its larger kin leaves behind.

Is Umbralisk a solo boss?

No. It has no legendary actions and a 4-PC party will out-act it, so it is not solo-capable. Run it as an add-on: pair it with a stronger leader, a spellcaster, or a pack of minions and let it work the front line.
