Who Is Venomojoy
Where the Bileblight passed, the wells ran green and the laughter began. Venomojoy was not born so much as accreted: a glutton-corpse that would not rot cleanly, swelling instead with every plague it drank until skin stretched drum-tight over a sea of bile. Folk of the moor-villages tell that it keeps a stolen cask lashed to its shoulder, brewing fresh contagion as it shambles, and that the single great eye weeps not tears but a thin venom that blights whatever it touches. Those who hear its wet, gurgling laughter are already breathing its sickness. Hunters who fell one learn the cruelest lesson last, for the thing bursts when slain, drowning its killers in the very rot it carried. It seeks low, damp places and the bones of the unburied, adding each new skull to the shifting cairn beneath its feet, a slow tide of pestilence wearing the shape of a grinning, bloated jester.
Run Venomojoy in Sixty Seconds
- Aura first. Anyone who touches Venomojoy, hits it in melee, or starts within 5 ft. eats 2d6 poison (DC 15 Con for none) — melee parties bleed just standing near it.
- Open with Spew Bile. If two or more PCs are clustered, the 15-ft. cone (7d6 poison, DC 15 Con) is the strongest opener; otherwise Multiattack the lowest-AC target.
- Kill-priority = the healer. Bite locks a target poisoned (disadvantage on attacks); aim it at whoever keeps the party standing.
- When bloodied, telegraph the burst. It wallows toward the casters — on death it ruptures for 6d6 poison in 15 ft. and leaves caustic difficult terrain, so warn melee to back off the kill.
- Three ways to run it: moor-stalker (lurches from bog, ambush aura), choke-plug (blocks a corridor, forces the cone), or boss add-on (soaks hits beside a flagship plague-lord).
Venomojoy
Large undead, neutral evil
CR 6 · 2,300 XP · GUIDELINEPlague-Bloated. Venomojoy is so swollen with corruption that its body is a reservoir of contagion. The first time a creature touches it, hits it with a melee attack, or starts its turn within 5 feet of it, that creature must succeed on a DC 15 Constitution saving throw or take 7 (2d6) poison damage.
Bilesplatter Burst. Signature. When Venomojoy drops to 0 hit points, its distended body ruptures. Each creature within 15 feet must make a DC 15 Dexterity saving throw, taking 21 (6d6) poison damage on a failed save, or half as much on a success. The area becomes difficult terrain coated in caustic slime until the end of the encounter.
Actions
Multiattack. Venomojoy makes one Spiked Gauntlet attack and one Bite attack.
Spiked Gauntlet. Melee Weapon Attack: +7 to hit, reach 10 ft., one target. Hit: 15 (2d10+4) bludgeoning damage plus 7 (2d6) poison damage.
Bite. Melee Weapon Attack: +7 to hit, reach 5 ft., one target. Hit: 11 (2d6+4) piercing damage, and the target must succeed on a DC 15 Constitution saving throw or be poisoned until the end of its next turn.
Spew Bile (Recharge 5-6). Venomojoy heaves the contents of the keg strapped to its shoulder in a 15-foot cone. Each creature in that area must make a DC 15 Constitution saving throw, taking 24 (7d6) poison damage on a failed save, or half as much on a success. A creature that fails is also poisoned until the end of its next turn.
Bonus Actions & Reactions
Wallow Forward. Reaction (when a creature ends its turn within 5 feet). Venomojoy lurches its bulk toward that creature, moving up to 5 feet. The creature must succeed on a DC 15 Strength saving throw or be knocked prone beneath its weight.
A corpulent, toxic-green horror oozing pustules and bile, one bulging eye fixed in a fanged, laughing grin. It drags a scavenged spiked gauntlet and a leaking war-keg of distilled rot across a litter of bleached skulls.
Venomojoy, Bound
Companion · acts on your initiative
Bound · GUIDELINEToxic Constitution (signature). Venomojoy is immune to poison damage and the poisoned condition, and has resistance to necrotic damage. While within 10 feet of its bonded ally, that ally has advantage on saving throws against poison and disease.
Actions
Caustic Slam. Melee Weapon Attack: +6 to hit, reach 5 ft., one target. Hit: 13 (2d8+4) bludgeoning damage plus 4 (1d8) poison damage.
Reactions
Soak the Blow. Reaction (when the bonded ally within 5 feet takes damage). Venomojoy's swollen flank absorbs the hit; it takes the damage instead, reduced by 5.
A smaller bile-spawn calved from Venomojoy's own body, devoted to shielding whatever master claimed its leash.

How It Fights — by Tier
Same creature, scaled threat. The traits never change; only the numbers move.
Open by wading into the tightest cluster so Plague-Bloated chips everyone, then Bite the squishiest body to lock down a healer with poison. Save Spew Bile for two or more lined-up targets. Pair it with a stronger plague-lord; do not run it alone against a party.
Open with Spew Bile on the packed front line, then Multiattack the lowest-AC target and let Plague-Bloated punish every melee swing against it. Kill-priority is the party healer (lock them poisoned). When bloodied it shoves toward the casters, baiting the burst on death.
Run it as a tanky add-on in a swamp or charnel lair, never as the lone threat. Position it to block a choke so the party must stand in its aura, then Spew Bile down the corridor. Pair it with a bigger boss and minions; its job is to soak hits and spread poison while the flagship does the killing.

How a bound creature acts
- A companion acts on its owner's initiative, immediately after the owner.
- It uses the Companion block above — roughly ×0.6 HP and damage, no recharge abilities, and at most one reaction trait.
- It moves freely; it takes its Action only while its owner is conscious.
- At 0 HP a companion is broken, not dead — restored on a long rest unless your table rules otherwise.

What CR is Venomojoy?
Venomojoy is CR 6 (2,300 XP) at its native Tier 2, built for level 5-10 parties. The stat block also scales down to a ~CR 2 Tier 1 version and up to a ~CR 11 Tier 3 version; the traits stay identical and only the numbers move.
Can Venomojoy be a companion or ally?
Venomojoy itself is an adversary, but it comes with a bound companion, Gurgle, a poison-immune bile-spawn that soaks blows for its master and grants advantage on poison and disease saves to a nearby ally. Use Gurgle when you want a durable, toxin-themed sidekick.
Is Venomojoy a solo boss?
No. It is a durable add-on monster with no legendary actions, so a four-PC party will out-act it. Run it alongside a bigger threat or a pack of minions in a lair, where its damaging aura and death-burst punish parties that bunch up to fight the real boss.
