Who Is Voidwrath Atrumos
They buried the last lord of a drowned dynasty in golden harness so that no grave-robber would mistake him for a pauper, and the gold proved his undoing. Greed loves a crowned corpse, and the spells laid to guard his barrow soured into something that would not let him rest. Now he walks as Atrumos, the Voidwrath, a revenant whose hate has outlived the kingdom that wronged him. His forked glaive was forged from a relic that drank the sun at an eclipse; where it cuts, warmth and memory leak out together, and the wound never wholly closes. He keeps the rites of his vanished court with terrible precision, demanding fealty from any who cross his cairn and answering refusal with the silent roar of the empty places between stars. Those who hear it dream ever after of a throne room with no walls, no torches, and no way out.
Run Voidwrath Atrumos in Sixty Seconds
- Hollow Roar first. If the party is bunched, open with the recharge scream — 4d8 necrotic plus frightened sets up your whole turn.
- Glaive eats max HP. Every Forked Glaive hit shaves the target's hit point maximum until a long rest; focus one PC to bury them.
- Kill-priority is radiant. A radiant hit or a crit blocks Undead Fortitude AND stops his Void-Drinking heal — make the cleric the threat he hunts.
- Reaping Step locks the line. Anyone who Disengages within 10 ft. still eats a free glaive swing; melee can't just peel off.
- Three ways to run him: grave-captain leading a skeleton honor guard, lone duelist who picks off stragglers in a corridor, or lair-anchored gatekeeper paired with a bigger undead boss.
Voidwrath Atrumos
Medium undead, lawful evil
CR 6 · 2,300 XP · GUIDELINEVoid-Drinking Blade. The forked glaive devours warmth and light. When Atrumos reduces a creature to 0 hit points or scores a critical hit, he regains 10 hit points and the area within 10 feet of him is briefly draped in darkness until the end of his next turn.
Undead Fortitude. If damage reduces Atrumos to 0 hit points, he makes a Constitution saving throw (DC 5 + the damage taken, max DC 18). On a success he drops to 1 hit point instead. Radiant damage or a critical hit prevents this.
Actions
Multiattack. Atrumos makes two Forked Glaive attacks.
Forked Glaive. Melee Weapon Attack: +7 to hit, reach 10 ft., one target. Hit: 11 (2d6+4) slashing damage plus 7 (2d6) necrotic damage, and the target's hit point maximum is reduced by an amount equal to the necrotic damage taken until it finishes a long rest.
Hollow Roar (Recharge 5-6). Atrumos lets out a soundless scream of the void. Each creature within 15 feet must succeed on a DC 14 Wisdom saving throw or take 18 (4d8) necrotic damage and be frightened until the end of its next turn; half damage and not frightened on a success.
Bonus Actions & Reactions
Reaping Step. Reaction When a creature Atrumos can see ends its move within 10 feet of him, he can make one Forked Glaive attack against it.

Bound · GUIDELINEGrave-Sworn (signature). While within 30 feet of an ally, Voidwrath Atrumos has advantage on saving throws against being frightened and cannot be turned by undead-affecting effects.

Notched Halberd. Melee Weapon Attack: +6 to hit, reach 10 ft., one target. Hit: 10 (1d10+5) slashing damage plus 3 (1d6) necrotic damage.
Reactions
Shield of the Fallen. Reaction When an ally Voidwrath Atrumos can see within 5 feet is hit by an attack, Voidwrath Atrumos adds 2 to that ally's AC against the triggering attack, possibly causing it to miss.
A lesser revenant of the same dead court, sworn to march at the Voidwrath's shoulder until the throne is restored — which is to say, forever.

How It Fights — by Tier
Same creature, scaled threat. The traits never change; only the numbers move.
Open by stalking the squishiest caster and forcing the Hollow Roar fright before melee. Use Reaping Step to punish anyone who tries to disengage. Pair him with a swarm of skeletons so the party can't focus him down.
Open Hollow Roar into a clustered party, then Multiattack the worst-armored target to stack hit-point-maximum reduction. Kill-priority: whoever can deal radiant damage, since it shuts off Undead Fortitude and his self-heal. Run him as the captain of an undead honor guard, not alone.
Hold him at the heart of his cairn-lair amid risen courtiers and let Reaping Step turn the room into a no-disengage zone. Open Hollow Roar, then carve down anyone who closed in. Pair him with a bigger threat — a wraith-king or a flagship lich — so his glaive does the line work while the boss casts.

How a bound creature acts
- A companion acts on its owner's initiative, immediately after the owner.
- It uses the Companion block above — roughly ×0.6 HP and damage, no recharge abilities, and at most one reaction trait.
- It moves freely; it takes its Action only while its owner is conscious.
- At 0 HP a companion is broken, not dead — restored on a long rest unless your table rules otherwise.

What CR is Voidwrath Atrumos?
He is built as a CR 6 elite undead warrior (Tier 2, AC 17, 119 HP). Scaled stat lines for Tier 1 (levels 1-4) and Tier 3 (levels 11-15) are provided so you can drop him into any campaign without rebuilding the block.
Can Voidwrath Atrumos be a companion or ally?
As written he is an adversary, but the bound revenant Mortbrand is statted as a playable ally or sidekick if you want a grave-sworn guardian. For Atrumos himself as a reluctant patron, reflavor Hollow Roar as a warning rather than an attack and have him demand a quest in exchange for his blade.
Is he meant to fight a party alone?
No — he is an add-on elite, not a solo boss. Without legendary actions a four-player party will out-act him, so field him alongside an undead honor guard or pair him with a larger flagship threat that anchors the encounter.
