The brimstone-cursed horror of the burning city · a boss your table can run with almost no prep. SRD 5.1-compatible.

Who Is Sulfire Wraith

When the cities of the plain burned beneath fire and brimstone, not every soul was permitted the mercy of death. The Sulfire Wraith is said to be the gathered guilt of those who looked back upon the destruction with longing rather than horror, their forms welded together by sulfur and sin into a single watching thing. Pilgrims who cross the salt-flats and ash-dunes speak of stalked eyes that open in the smoke long before the body emerges, and of a heat that strips reactions and resolve alike. It cannot speak, yet it remembers every tongue it once knew, and it is drawn to those who carry secret cruelties, as though it recognizes its own kind. To meet its withering gaze is to feel the fire of judgment rekindled in the marrow. Salt blessed by the faithful is said to slow it; sunlight blinds it; but nothing has yet been recorded that lays it permanently to rest.

Sulfurous Aura. Acrid brimstone smoke hangs around the wraith in a 10-foot radius. The area is lightly obscured to creatures other than the wraith, and a creature that starts its turn there must succeed on a DC 15 Constitution saving throw or be unable to take reactions until the start of its next turn.

Many Watching Eyes. The wraith's writhing stalks cannot be surprised and it has advantage on Wisdom (Perception) checks. It is aware of every creature within 60 feet that is not behind total cover, even invisible ones.

Sunlight Sensitivity. While in sunlight, the wraith has disadvantage on attack rolls and on Wisdom (Perception) checks that rely on sight.

Actions

Multiattack. The wraith makes one Maw attack and two Clawed Limb attacks.

Maw. Melee Weapon Attack: +7 to hit, reach 5 ft., one target. Hit: 14 (2d10 + 4) piercing damage plus 7 (2d6) necrotic damage, and the target must succeed on a DC 15 Constitution saving throw or its hit point maximum is reduced by an amount equal to the necrotic damage taken until it finishes a long rest.

Clawed Limb. Melee Weapon Attack: +7 to hit, reach 10 ft., one target. Hit: 11 (2d6 + 4) slashing damage.

Bonus Actions & Reactions

Searing Eye Ray (Reaction). When a creature the wraith can see ends its turn within 60 feet, the wraith lashes a stalk and unleashes a ray at it. The target makes a DC 15 Dexterity saving throw, taking 18 (4d8) fire damage on a failed save, or half as much on a success. A creature reduced to 0 hit points by this damage is reduced to ash.

Legendary Actions (3/Round)

Eye-Stalk Lash. The wraith makes one Clawed Limb attack.

Withering Gaze (Costs 2 Actions). One creature the wraith can see within 60 feet must succeed on a DC 15 Wisdom saving throw or be frightened of the wraith until the end of its next turn, and it takes 9 (2d8) necrotic damage.

Brimstone Burst (Costs 2 Actions). The wraith vents fire. Each creature within 10 feet must make a DC 15 Dexterity saving throw, taking 14 (4d6) fire damage on a failed save, or half as much on a success.

A crouching, four-limbed abomination of seared flesh, its skull split by a fanged maw and a glistening exposed brain, while a crown of eye-tipped stalks coils and watches from every angle.

Watcher's Warning (signature). While Sulfire Wraith is conscious, allies within 30 feet of it can't be surprised and have advantage on saving throws against being blinded.

Actions

Ember Ray. Ranged Spell Attack: +6 to hit, range 60 ft., one target. Hit: 12 (3d6 + 2) fire damage.

Reactions

Smother the Flame. When an ally within 30 feet takes fire damage, Sulfire Wraith can use its reaction to reduce that damage by 1d10.

A lone severed eye-stalk that crawls on root-like tendrils, its iris burning low, leashed to a master who learned the wraith's true name.

Lead with Brimstone Burst when two or more PCs cluster, then split the maw and claws across the front line. Spend reactions on the Searing Eye Ray against ranged attackers ending their turns at distance. Kill-priority is any healer feeding the target whose hit point maximum you're draining.

Run it as a true solo centerpiece: open with Withering Gaze on the controller, Multiattack the tank, and burn legendary actions on Eye-Stalk Lash plus a Brimstone Burst when melee crowds in. The Sulfurous Aura plus Searing Eye Ray punishes anyone who ends a turn near or in sight of it, so force the party to spread out and chase. Save the second Eye Ray reaction for the lowest-HP caster to ash them outright.

What CR is Sulfire Wraith?

It is built as a Challenge Rating 10 solo centerpiece (5,900 XP) at its native Tier 3. Because its legendary actions and the Searing Eye Ray reaction give it real action economy, the sheet sits one band below raw Tier 3 numbers so a level 11-15 party of four still gets a fair, deadly fight. Scaled Tier 1 and Tier 2 versions are provided for lower-level tables.

Can Sulfire Wraith be a companion or ally?

The wraith itself is far too large and malevolent to befriend, but a severed, leashed eye-stalk called Cinder-Eye is provided as a companion statblock (AC 13, 85 HP) that warns allies of ambush, fires an Ember Ray, and can smother incoming fire damage for whoever has learned the wraith's true name.

Is it immune to fire?

Yes. Born from brimstone, it is immune to fire and poison damage and to the charmed, frightened, exhaustion, and poisoned conditions, and it resists necrotic damage. Lean on radiant, force, cold, or thunder damage, and remember it has Sunlight Sensitivity — fighting it in daylight gives it disadvantage on attacks and sight-based Perception.
