Lord of the Flies, first-fallen monarch of the buzzing Abyss · a boss your table can run with almost no prep. SRD 5.1-compatible.

Who Is Beelzebub

Among the first three angels to strike the Abyss and wake in ruin, Beelzebub rose beside Lucifer and Samael as a founding monarch of Hell, and the scars the archangels left upon him he wears yet as a crown of antler and flame. Where his rivals took dominion over war and pride, Beelzebub claimed the swarm: he is the Lord of the Flies, sovereign of decay, and every rot and pestilence that creeps across the lower planes answers the drone of his wings. Petitioners who beg his favor learn that his counsel is sweet and his price is corruption, for he prefers to win by patience, letting an enemy sicken and crumble before a blow is ever struck. His court buzzes without cease, and those who stand long in his presence find their lungs full of wings. To name him is to be heard; to bargain with him is to already be losing.

Cloud of Flies. A droning swarm boils around Beelzebub. At the start of each of his turns, every creature within 10 feet of him takes 7 (2d6) poison damage. While the cloud persists, attack rolls against Beelzebub by creatures within 10 feet are made with disadvantage as the flies blind and harry them.

Magic Resistance. Beelzebub has advantage on saving throws against spells and other magical effects.

Innate Spellcasting. His spellcasting ability is Charisma (spell save DC 16). He can innately cast the following spells, requiring no material components: at will command, insect plague; 3/day each fear, stinking cloud; 1/day insect plague upcast (already listed at will, treat extra casts as 1/day each contagion).

Actions

Multiattack. Beelzebub makes three attacks: one with his Flaming Scepter and two with his Stinging Lash.

Flaming Scepter. Melee Weapon Attack: +9 to hit, reach 10 ft., one target. Hit: 16 (3d6+5) bludgeoning damage plus 10 (3d6) fire damage.

Stinging Lash. Melee Weapon Attack: +9 to hit, reach 15 ft., one target. Hit: 12 (2d6+5) piercing damage, and the target must succeed on a DC 16 Constitution saving throw or be poisoned until the end of its next turn.

Bonus Actions & Reactions

Swarmstep (Bonus Action). Beelzebub dissolves into a column of flies and reforms in an unoccupied space he can see within 30 feet. This movement does not provoke opportunity attacks.

Buzzing Recoil (Reaction). When a creature within 10 feet hits Beelzebub with a melee attack, the cloud of flies lashes back: the attacker takes 9 (2d8) poison damage.

Legendary Actions (3/Round)

Detect. Beelzebub makes a Wisdom (Perception) check.

Lash. Beelzebub makes one Stinging Lash attack.

Spreading Pestilence (Costs 2 Actions). Beelzebub points his scepter at a point he can see within 60 feet. Each creature within 10 feet of that point must succeed on a DC 16 Constitution saving throw or take 14 (4d6) poison damage and be poisoned until the end of its next turn.

A gaunt, antlered monarch crowned in fire, robed in tattered carapace, his torso unfurling into great cicada-veined wings while his lower body coils away into a writhing knot of serpents and maggots set upon a base of Abyssal flame.

Hive Sense (signature). Beelzebub shares the senses of every fly within 60 feet and cannot be surprised. While within 30 feet of Beelzebub, both have advantage on Wisdom (Perception) checks.

Actions

Barbed Proboscis. Melee Weapon Attack: +8 to hit, reach 10 ft., one target. Hit: 14 (3d6+4) piercing damage plus 7 (2d6) poison damage.

Reactions

Drone Ward. When an ally within 5 feet is hit by an attack, Beelzebub interposes its wing-shield, reducing the damage by 9 (2d8).

A man-sized horror of fused chitin and humming wings, chained to its master's will, that throws itself between Beelzebub and any blade.

Open with Cloud of Flies softening the front line, then Swarmstep to the squishiest target and lash it. Kill-priority: drop whoever is concentrating or healing. Far too lethal raw at this tier, so run him as a scaled-down nightmare or a dream-vision, not a fair fight.

Lead with insect plague to wall off the casters, then Multiattack the nearest melee. Use Swarmstep to stay off the front and let the poison tick. Pair him with a couple of lesser devils so the party can't simply gang the boss.

Open with Cloud of Flies up and insect plague centered on the casters, then Multiattack and Swarmstep to never be flanked. Spend legendary actions on Spreading Pestilence into clusters; when bloodied, retreat into the flies and force them to come to you through the poison. Kill-priority: anyone with mass-cure or fire vulnerability.

What CR is Beelzebub?

Natively CR 13 (Tier 3, ~level 11-15), built as a solo boss with legendary actions. The tiers block also gives scaled CR 2 (level 1-4) and CR 6 (level 5-10) versions; the traits and actions stay identical, only AC, HP, to-hit, save DC, and damage shift to the band.

Can Beelzebub be a companion or ally?

Not directly — he is a Goetic monarch and a boss-tier adversary, not an ally. If players strike an Abyssal bargain, give them Sythrael, the Bound Drone (his attendant horror) as a temporary summoned ally instead, using the companion block.

How do I keep the fly aura from grinding the table to a halt?

Roll the Cloud of Flies poison once for all creatures in range at the start of his turn and apply it together, and remind melee players that the disadvantage is the trade-off for closing in. It is a steady drip, not a save each turn, so it stays fast.
