Who Is Lilithra The Abyssal Queen
Of the spirits abroad in Sodom and Gomorrah, none is so patient in her appetite as Lilithra, named the Abyssal Queen by those few who fled her embrace. She wears no crown of metal but one of grown horn, and her face is hidden behind a mask shaped like a moth, for she is drawn to every light of pride and pleasure and snuffs it in the dark of her coils. Where lesser demons rend and feast in haste, Lilithra wraps the boastful, the lustful, and the cruel within her braided flesh and lets them learn slowly that they were never the predator. The second face set in her breast whispers comforts in the voice of whatever the victim most loved, so that many cease to struggle and are crushed smiling. Scholars of the seventy-two warn that she answers summons readily, eager for new throats to fill her endless, embracing hunger.
Run Lilithra The Abyssal Queen in Sixty Seconds
- She is a grappler, not a chaser. Open by constricting the most mobile or most fragile PC and dragging them out of the formation; her whole threat is the hold.
- Kill-priority: enemy escape tools. Anyone who can free or teleport a captive (Misty Step, Freedom of Movement, a knock-prone bruiser) goes to the top of her list.
- When bloodied, she stops trading claws and commits everything to crushing whoever she already holds with Constrict plus legendary Smother every round.
- Three ways to run her: (1) Lone abyssal horror in a flesh-pit lair; (2) flagship with Vael guarding her flank; (3) trim to Tier 1-2 numbers and use her as the elite add-on under a bigger Goetia boss.
- Don't forget legendary actions. Slither to reposition with captives in tow, second Molten Claw at the cost of 2, and Smother to murder a grappled PC between their friends' turns.

Large fiend (demon), chaotic evil
CR 11 · 7,200 XP · GUIDELINEWrithing Mass. Lilithra's body is a knot of grasping coils. A creature she has grappled has disadvantage on ability checks made to escape, and she can move at full speed while dragging up to two grappled creatures.
Magic Resistance. Lilithra has advantage on saving throws against spells and other magical effects.
Maddening Caress. Signature. Lilithra exudes a smothering, intimate dread. At the start of each of her turns, each creature grappled by her must succeed on a DC 16 Wisdom saving throw or be frightened of her until the start of its next turn. While frightened this way, the creature treats Lilithra's coils as difficult terrain even if it breaks free.
Actions
Multiattack. Lilithra makes two Molten Claw attacks and uses Constrict if a hand is free.
Molten Claw. Melee Weapon Attack: +9 to hit, reach 10 ft., one target. Hit: 15 (2d8+6) slashing damage plus 9 (2d8) fire damage.
Constrict. Melee Weapon Attack: +9 to hit, reach 10 ft., one Medium or smaller creature. Hit: 18 (3d8+5) bludgeoning damage, and the target is grappled (escape DC 16) and restrained until the grapple ends. Lilithra can grapple up to two creatures at once.
Bonus Actions & Reactions
Barbed Lash. Reaction. When a creature within 10 ft. that Lilithra can see hits her with a melee attack, she lashes a hooked coil at it. The attacker must succeed on a DC 16 Dexterity saving throw or take 9 (2d8) piercing damage and be pulled 5 ft. toward her.
Legendary Actions (3/Round)
Legendary Actions. Lilithra can take 3 legendary actions, choosing from the options below. Only one can be used at a time and only at the end of another creature's turn. She regains spent actions at the start of her turn.
Slither. Lilithra moves up to half her speed without provoking opportunity attacks, dragging any creatures grappled by her along with her.
Molten Claw (Costs 2 Actions). Lilithra makes one Molten Claw attack.
Smother. One creature grappled by Lilithra must succeed on a DC 16 Constitution saving throw or take 13 (3d8) bludgeoning damage as her coils crush inward.
A tower of braided crimson sinew rises into a horned, butterfly-masked head, a second demon-face leering from its torso while molten claws drip embers. She does not chase prey; she folds it into herself.

Bound · GUIDELINEBound Devotion (signature). While within 30 ft. of Lilithra, Lilithra The Abyssal Queen has advantage on saving throws against being frightened, and the first time each round Lilithra grapples a creature, Lilithra The Abyssal Queen can move up to 10 ft. toward that creature as a free action.
Actions
Rending Talons. Melee Weapon Attack: +7 to hit, reach 5 ft., one target. Hit: 13 (2d8+4) slashing damage, and if the target is grappled, the attack deals an extra 7 (2d6) damage.
Reactions
Shield the Queen. Reaction. When an attack hits Lilithra while Lilithra The Abyssal Queen is within 5 ft. of her, Lilithra The Abyssal Queen interposes and the attack hits Lilithra The Abyssal Queen instead.
A lithe, half-melted demon who orbits the Queen's coils, finishing what she begins and dying gladly between her and any blade.

How It Fights — by Tier
Same creature, scaled threat. The traits never change; only the numbers move.
Open by grappling the closest squishy striker and dragging them away from healers. If foes cluster, snake between them so two are in claw reach at once. Kill-priority is whoever broke free last round; she hates losing a hold.
Constrict the party's mobility piece first, then Smother it every round while clawing a second target. If clustered, position so Barbed Lash punishes melee. Bring Vael or a bigger flagship; at this tier she reads as the captain, not the final boss.
Centerpiece boss: grapple two PCs turn one, frighten them with Maddening Caress, then spend legendary Smother to grind them between turns. Kill-priority is whoever can teleport allies out of her coils. Use Slither to keep dragging captives away from rescue.

How a bound creature acts
- A companion acts on its owner's initiative, immediately after the owner.
- It uses the Companion block above — roughly ×0.6 HP and damage, no recharge abilities, and at most one reaction trait.
- It moves freely; it takes its Action only while its owner is conscious.
- At 0 HP a companion is broken, not dead — restored on a long rest unless your table rules otherwise.

Her native build is CR 11 (7,200 XP) for an 11th-15th level party, designed as a solo-capable boss with legendary actions. Scaled stat blocks for Tier 1 (~CR 2) and Tier 2 (~CR 6) are provided so she can headline lower-level tables too.
Can Lilithra The Abyssal Queen be a companion or ally?
She is built as an adversary boss, but a bargain-struck warlock or a Sodom-aligned villain could field her as an ally. If she fights beside players, drop her to native-tier numbers and have her prioritize grappling enemy frontliners while the party works the flanks.
How do I keep her from being a one-trick grapple wall?
Lean into the horror: use Maddening Caress to frighten captives, the torso-face to taunt in a loved one's voice, and Barbed Lash to drag would-be rescuers in. Move her around with Slither so the fight isn't static, and let captives feel her coils slowly tightening rather than instantly bursting them.
