Who Is Akivasha
Of Akivasha the chronicles speak in lowered voices, for she was a princess of a dynasty that crumbled to dust ten centuries gone, and yet she walks. Loath to surrender her beauty to the grave, she struck a bargain with a thing that festers beneath the old temples, and the bargain held: she does not age, she does not die, she does not love. She holds court in a vaulted crypt-palace where the demon-faced throne watches with carved eyes, and there she takes the young and the comely as lovers, draining them slow across the long dark so that they wither into worship and then into corpses. The serpent that coils at her wrist is no familiar but a fragment of her patron, ever-whispering. Steel that has not been blessed parts before her unharmed flesh; only holy fire and the cleansing light unmake what she has become. Travelers who glimpse her swear she is the loveliest woman alive. They are half right.
Run Akivasha in Sixty Seconds
- She is a charmer, not a wall. Akivasha wants the party fighting each other, not her. Lead every round with a charm and let her thralls soak the swings.
- Kill-priority: radiant. Her Regeneration only stops when she takes radiant damage, and radiant is the only clean way to drop her for good. Whoever has it dies first; everyone else just delays her.
- When bloodied, she feeds. At half HP use Queen's Step to slip behind cover and Blood Drain a charmed or held victim, clawing back 14+ HP and shrinking a PC's max. She is healthiest right when the party thinks she's losing.
- Three ways to run her: Seductress (open with charms and illusions, fight from behind thralls), Predator (grapple-and-drain a lone PC to nova-heal), or Queen (full legendary boss with animate dead minions in a throne-room set piece).
- Undying Beauty punishes attackers. Remind hitters that landing a blow can charm them and halve their own damage — they may want to use spells and ranged attacks that bypass her grace.
Akivasha
Medium undead (shapechanger), neutral evil
CR 6 · 2,300 XP · GUIDELINEUndying Beauty. Signature. Akivasha's drained centuries have left her flawless. When a creature she can see hits her with an attack, she may force it to make a DC 15 Wisdom save; on a failure it is charmed until the end of its next turn and that attack's damage is halved as it falters. A creature can be affected this way only once per turn.
Regeneration. Akivasha regains 10 hit points at the start of her turn if she has at least 1 hit point and has taken no radiant damage since her last turn. If reduced to 0 hit points by non-radiant damage, she instead drops to 1 hit point and falls dormant until she feeds.
Spellcasting. Akivasha casts these spells, requiring no material components (spell save DC 14, +6 to hit): At will: chill touch, charm person, minor illusion. 3/day: hold person, vampiric touch. 1/day: fear, animate dead.
Actions
Multiattack. Akivasha makes two attacks, only one of which can be a Blood Drain.
Raking Talons. Melee Weapon Attack: +6 to hit, reach 5 ft., one target. Hit: 9 (2d6+2) slashing damage.
Blood Drain. Melee Weapon Attack: +6 to hit, reach 5 ft., one creature that is grappled, restrained, incapacitated, or willing. Hit: 14 (3d6+4) necrotic damage. The target's hit point maximum is reduced by an amount equal to the damage taken, and Akivasha regains that many hit points. The reduction lasts until the target finishes a long rest. The target dies if this reduces its maximum to 0.
Bonus Actions & Reactions
Beguiling Glance (Bonus). Akivasha targets one creature within 30 ft. that can see her. The target must succeed on a DC 14 Wisdom save or be charmed by her until the end of her next turn, regarding her as a trusted friend.
Serpent Coil (Reaction). When a creature within 5 ft. moves away or attacks an ally, the conjured serpent at her wrist strikes: +6 to hit, Hit: 7 (2d6) poison damage and the target's speed is halved until the end of its next turn.
Legendary Actions (3/Round)
Legendary Actions. Akivasha can take 3 legendary actions, choosing from the options below. Only one can be used at a time and only at the end of another creature's turn. She regains spent legendary actions at the start of her turn.
Whispered Lie. Akivasha casts charm person or minor illusion, or makes a Deception check to feint.
Talons. Akivasha makes one Raking Talons attack.
Queen's Step (Costs 2 Actions). Akivasha moves up to her speed without provoking opportunity attacks and may make one Blood Drain attack against a creature she ends adjacent to.
A slender, near-nude queen in a horned diadem stands before a leering demon throne, one hand raised so a conjured serpent rises to her fingertips, her hair spilling like spilled wine.
Akivasha, Bound
Companion · acts on your initiative
Bound · GUIDELINEThrall's Devotion (signature). Akivasha automatically fails saves against effects from the creature he is bound to and will throw himself between her and harm, imposing disadvantage on the first attack against her each round if he is within 5 ft. of her.
Actions
Guard's Blade. Melee Weapon Attack: +5 to hit, reach 5 ft., one target. Hit: 8 (1d8+4) slashing damage.
Reactions
Take the Blow. When his mistress within 5 ft. is hit by an attack, Akivasha swaps places and takes the damage himself.
A hollow-eyed captain who was handsome once, now Akivasha's drained and doting bodyguard, fighting on past all reason to keep her safe.

How It Fights — by Tier
Same creature, scaled threat. The traits never change; only the numbers move.
Open with Beguiling Glance to peel off the strongest melee attacker, then claw the casters. Kill-priority is whoever carries radiant or holy fire, since they shut down her regeneration. Pair her with a few crypt-thralls so the party can't simply dogpile her.
Open by charming the front-liner with Beguiling Glance, then hold the cleric with hold person and Blood Drain a paralyzed victim to heal and shrink their HP max. When bloodied, spend Queen's Step to retreat through her thralls and feed at the edge. Anyone packing radiant damage is your number-one kill target.
Lead with fear to scatter the line, then legendary-action Talons and Whispered Lie between turns so she never stops pressuring. Focus Blood Drain on whoever can dispel her charms or smite for radiant. Surround her with raised dead via animate dead so the party must fight through the throne room to reach the queen.

How a bound creature acts
- A companion acts on its owner's initiative, immediately after the owner.
- It uses the Companion block above — roughly ×0.6 HP and damage, no recharge abilities, and at most one reaction trait.
- It moves freely; it takes its Action only while its owner is conscious.
- At 0 HP a companion is broken, not dead — restored on a long rest unless your table rules otherwise.

What CR is Akivasha?
Her native build is CR 6 (2,300 XP), tuned for a tier-2 party of levels 5-10. She is solo-capable thanks to legendary actions and Regeneration, so she can headline a single encounter; the sheet already drops a band to keep her fair as a boss. Scale up to roughly CR 11 at tier 3 or down to about CR 2 at tier 1 using the tier table.
Can Akivasha be a companion or ally?
Rarely, and never trustworthy. A desperate party might strike a bargain with her, but she keeps faith only as long as it feeds her — expect betrayal the moment a fresh, comelier victim appears. Run her as a manipulative patron rather than a loyal ally. Her bound thrall Lurén is the truly devoted bodyguard at her side.
How do I stop her from healing forever?
Deal radiant damage. Her Regeneration shuts off for a round whenever she takes radiant, and reducing her to 0 with radiant actually destroys her — otherwise she only drops dormant and rises again after feeding. Holy water, smites, and clerics are her bane; deny her grappled or helpless targets and she can't Blood Drain to heal.
