Ciacco — The Encounter

The Hog of Florence, drowned glutton with many mouths · a boss your table can run with almost no prep. SRD 5.1-compatible.

Who Is Ciacco

In the third circle the wretched lie under a cold, endless slop of rain, hail and black snow, and over them the worm-king worries their swollen bodies. Among the drowned shades rose one called Ciacco, the Hog, whom Florence knew in life as a man who lived for nothing but his belly. Gluttony has not ended with his death; it has consumed him entire. The shade has bloated into a mountain of flesh, its skin split open by new mouths that hunger where the first could not be filled. It can no longer speak its old prophecies, only chew. Travellers who wade the stinking marsh report a low wet grinding before they ever see it, and by then the muck itself has thickened to hold their feet. To feed Ciacco is to vanish without sound, drawn down into a softness that closes over the head like water. It does not chase. It does not need to. The filth brings supper to its many waiting mouths.

Many Mouths. Ciacco's body is split by several fanged maws. It does not need to see a creature to bite it and cannot be flanked. While it has a creature grappled, it can still bite a different target with another mouth.

Gorged Mass. Ciacco's space is difficult terrain for other creatures, and it cannot be knocked prone or moved against its will by any effect that does not deal force damage. The Hog of Florence does not fall; it merely settles deeper into the filth.

Actions

Multiattack. Ciacco makes two Devouring Bite attacks. It can replace one bite with a use of Engulf if it has a target grappled.

Devouring Bite. Melee Weapon Attack: +7 to hit, reach 5 ft., one target. Hit: 16 (3d8+3) piercing damage, and if the target is Medium or smaller it is grappled (escape DC 15). Until this grapple ends the target is restrained and Ciacco cannot bite with that same mouth.

Engulf. A creature Ciacco is grappling must make a DC 15 Constitution saving throw. On a failure it takes 14 (4d6) acid damage and is swallowed; on a success it takes half damage and is shoved 5 feet away. A swallowed creature is blinded, restrained, has total cover, and takes 10 (3d6) acid damage at the start of each of Ciacco's turns. Ciacco can have one creature swallowed at a time. If Ciacco takes 20 damage or more on a single turn from the swallowed creature, it must make a DC 14 Constitution save at the end of that turn or regurgitate the creature into an unoccupied space within 5 feet, which falls prone.

Bonus Actions & Reactions

Wallowing Lunge. Reaction (recharges after Ciacco swallows a creature): When a creature within 5 feet stands up from prone, Ciacco makes one Devouring Bite against it.

A vast hill of greasy, sweating flesh slumps in the muck, its hide cracked open by hungry fanged mouths that never stop chewing. Where a face should be, only another maw gapes, weeping filthy rain.

Insatiable (signature). When the Glutton-Shade reduces a creature to 0 hit points or finishes a meal, it gains 10 temporary hit points and its next bite this combat deals an extra 1d8 damage.

Actions

Ravening Bite. Melee Weapon Attack: +5 to hit, reach 5 ft., one target. Hit: 11 (2d6+4) piercing damage, and the target cannot regain hit points until the start of its next turn.

Reactions

Feast on the Fallen. When a creature within 5 feet drops to 0 hit points, the Glutton-Shade moves up to 10 feet toward it without provoking opportunity attacks.

A lesser drowned shade, leaner than its master but no less hungry, kept at heel to drag the slow and the wounded down into the mire.

Open by lurching into the densest knot of party and biting the squishiest body to grapple it. Kill-priority is the grappled victim, since Engulf can swallow a low-tier PC fast. Pair it with a ranged threat so the party can't just kite this slow mound; on its own it is a terrain puzzle the rogue solves.

Open with two bites, grapple a martial, then Engulf on round two to remove them from the fight. The swallowed PC must out-damage 20/turn to be vomited up, so the party's clock is the swallowed friend. This is the gluttony piece of an Inferno table, not the boss; pair it with a faster devil that punishes anyone rushing to free the meal.

Now a full hazard-monster: bite, swallow, and let Engulf's 3d6 acid grind a high-tier PC while the others wade difficult terrain to reach it. Kill-priority for the party is freeing the swallowed character before two failed turns end them. Deploy as the static anchor of an encounter; let mobile minions herd the party into Ciacco's reach.

What CR is Ciacco?

Native CR 6 (Tier 2, ~levels 5-10): AC 15, 138 HP, +7 to hit, save DC 15. The block also scales to a CR 2-style version for Tier 1 and a CR 11-style version for Tier 3 by swapping only the numbers in the tier table. It is not solo-capable, run it as an add-on, not a lone boss.

Can Ciacco be a companion or ally?

Ciacco itself is a hazard, not an ally, but the bound Gorging Glutton-Shade (AC 12, 82 HP) is its companion stat block, an insatiable lesser shade that bites, blocks healing, and lunges on the fallen. Use it as a leashed enforcer for a fiendish or undead patron.

How do you escape being swallowed by Ciacco?

A swallowed creature is blinded, restrained, and takes 3d6 acid each of Ciacco's turns. If the whole party deals 20+ damage to Ciacco in a single turn while the creature is inside, Ciacco must make a DC 14 Constitution save or vomit the victim into an adjacent space (prone). Otherwise, kill Ciacco or reduce it to 0 to free the meal.
