Who Is Ometeotl
Before the smoke and before the sun, the Twofold Lord dwelt alone in the thirteenth and highest heaven, neither wholly male nor wholly female but both folded into one breathing word. The grimoires of the feathered priests name it Ometecuhtli and Omecihuatl in a single mouth, the source from which all gods and the four directions were spoken into being. It does not war as lesser powers war; it decrees, and what is decreed becomes. Pilgrims who climb to its enthroned ring find the air thick with the scent of copal and the certainty that they are being measured. Those it favors leave gifted with names that open doors in the underworld; those it judges wanting are simply unmade, their threads quietly withdrawn from the great loom. It rarely descends, for to leave the throne is to risk the unraveling of all it has authored. When it does, the sky itself bends to make room.
Run Ometeotl in Sixty Seconds
- Aspect every turn First thing each turn, pick Lord (extra radiant on attacks) or Lady (regain 10 HP + a foe gets disadvantage on its next save). Default to Lord while healthy, Lady once bloodied.
- Open with Word of Becoming Lead with the 30-ft. stun cone the moment two or more PCs cluster; 33 psychic plus a stun reshapes the fight before melee even starts.
- Kill-priority: the healer Use the Serpent Lash's pull and the Decree legendary to drag and frighten the party's support out of position, then Multiattack it down.
- When bloodied Flip to Lady aspect and spend Shift Aspect legendary actions to stack regeneration; it can out-heal a party that splits its damage.
- Three ways to run it As a god, solo with all legendary actions live; as a temple boss, drop the band and add serpent cultists; as a puzzle, let players sabotage the enthroned ring to strip Throne of the Thirteenth Heaven.
Ometeotl
Large celestial, neutral
CR 11 · 7,200 XP · GUIDELINEDual Nature. Ometeotl is at once Ometecuhtli and Omecihuatl, the male and female halves of one being. At the start of each of its turns, it chooses an aspect: Lord (its weapon and Serpent Lash deal an extra 7 (2d6) radiant damage) or Lady (it regains 10 hit points and one creature it can see within 60 ft. has disadvantage on the next saving throw it makes before the start of Ometeotl's next turn).
Throne of the Thirteenth Heaven. Ometeotl has advantage on saving throws against spells and other magical effects. While within 20 ft. of its enthroned ring, it cannot be moved against its will and ignores difficult terrain.
Actions
Multiattack. Ometeotl makes two attacks: one with its Serpent Staff and one with its Serpent Lash. It can replace one attack with a use of Word of Becoming if it is available.
Serpent Staff. Melee Weapon Attack: +9 to hit, reach 10 ft., one target. Hit: 18 (3d8 + 4) bludgeoning damage plus 9 (2d8) poison damage.
Serpent Lash. Melee Weapon Attack: +9 to hit, reach 15 ft., one target. Hit: 14 (3d6 + 4) slashing damage, and the target must succeed on a DC 16 Strength saving throw or be pulled up to 10 ft. toward Ometeotl.
Word of Becoming (Recharge 5-6). Ometeotl speaks a syllable of the first creation. Each creature of its choice within 30 ft. must make a DC 16 Wisdom saving throw. On a failure, a creature takes 33 (6d10) psychic damage and is stunned until the end of its next turn; on a success, it takes half damage and is not stunned.
Bonus Actions & Reactions
Mirror of Smoke (Reaction). When a creature Ometeotl can see hits it with an attack, it can impose disadvantage on that attack roll. If the attack then misses, Ometeotl may immediately move up to half its speed without provoking opportunity attacks.
Legendary Actions (3/Round)
Serpent Strike. Ometeotl makes one Serpent Staff or Serpent Lash attack.
Shift Aspect (Costs 2 Actions). Ometeotl changes its Dual Nature aspect and regains 10 hit points.
Decree (Costs 2 Actions). Ometeotl points at one creature it can see within 60 ft. That creature must succeed on a DC 16 Charisma saving throw or be frightened of Ometeotl until the end of its next turn and take 11 (2d10) radiant damage.
A twofold deity sits crowned in plumes upon a stepped-pyramid throne, a living serpent-staff across its knees, suspended forever inside a turning ring of carved heavens beneath a blazing solar disc.

Companion · acts on your initiative
Bound · GUIDELINESustaining Word (signature). At the start of its turn, one ally within 30 ft. of Ometeotl regains 7 (2d6) hit points and ends one effect causing it to be frightened.
Actions
Maize Sickle. Melee Weapon Attack: +6 to hit, reach 5 ft., one target. Hit: 11 (2d6 + 4) slashing damage plus 7 (2d6) radiant damage.
Reactions
Intercede. When an ally within 10 ft. is hit by an attack, Ometeotl can grant that ally +3 AC against the triggering attack, possibly causing it to miss.
The Lady's softer half made flesh, a serene maize-crowned figure who nourishes the faithful even as the Lord pronounces their fate.

How It Fights — by Tier
Same creature, scaled threat. The traits never change; only the numbers move.
Open with Word of Becoming to scatter and stun the cluster, then wade in with the Serpent Lash to drag the lowest-AC striker into reach. Kill-priority is whoever is buffing the party. Run it as a temple guardian, not a god, and pair it with cultist adds so the party can't simply focus it down.
Sit on the ring, hold Word of Becoming until two or more PCs bunch up, then alternate Lord aspect for burst and Lady aspect when bloodied to claw HP back. Lash the healer out of formation and focus it. Pair with a serpent-faction lieutenant so the boss isn't alone.
Run it solo as a centerpiece deity. Open in Lord aspect, Multiattack the frontline, and spend legendary actions on Serpent Strike between turns; flip to Lady aspect the moment it drops below half to out-heal incoming damage. Save Mirror of Smoke for the party's biggest hit and use Decree to peel the cleric. Kill-priority for the party is its healing output, so make them choose between stopping the Lady and surviving the Lord.

How a bound creature acts
- A companion acts on its owner's initiative, immediately after the owner.
- It uses the Companion block above — roughly ×0.6 HP and damage, no recharge abilities, and at most one reaction trait.
- It moves freely; it takes its Action only while its owner is conscious.
- At 0 HP a companion is broken, not dead — restored on a long rest unless your table rules otherwise.

What CR is Ometeotl?
Its home tier is CR 11 (Tier 3, levels 11-15): AC 18, 184 HP, +9 to hit, save DC 16. Because it is solo-capable with legendary actions, the sheet already plays up a band, so a balanced party of four should be level 11 or higher. Tier 1 and Tier 2 sheets are provided to scale it down as a lesser temple guardian.
Can Ometeotl be a companion or ally?
The deity itself is a boss, not a follower, but its bound aspect Tonacacihuatl, Echo of the Lady (AC 15, 110 HP) makes an ideal divine ally or patron-sent guardian: she sustains the party with Sustaining Word, strikes with the Maize Sickle, and can Intercede to shield a falling companion.
Is Ometeotl drawn from real mythology?
Yes. Ometeotl is the dual creator deity of Aztec religion, Ometecuhtli and Omecihuatl in one being, dwelling in Omeyocan, the thirteenth heaven. This is public-domain Mesoamerican mythology, and the stat block leans on that source rather than any proprietary cosmology.
