Who Is Ophanim

Of the wheels beside the living creatures the prophets wrote plainly: their appearance was as a wheel within a wheel, their rims so high they were dreadful, and the rims were full of eyes round about. The ophanim are not steeds nor servants but a tier of the heavenly host given form — wheels of fire that move wherever the spirit wills, never turning as they go. Where the seraphim sing and the cherubim guard the throne, the ophanim are the throne's motion, carrying judgment from the firmament to the dust without ever bending a course. To stand before one is to be seen entirely, from every angle at once, by a thing that has gazed upon glory and will not soften its light for mortal eyes. Sacrifices were offered less in worship than in plea: that the wheel might pass over and not pause. When it pauses, it is because a reckoning has come due, and the fire that wreathes it does not warm — it weighs.

Eyes Without Number. The ophanim's rings are studded with countless watching eyes. It can't be surprised, has advantage on saving throws against being blinded, and any creature that starts its turn within 30 feet of the ophanim and can see it must succeed on a DC 16 Wisdom saving throw or have disadvantage on the first attack roll it makes that turn, unnerved by the gaze that follows from every angle.

Whirling Flight. The ophanim moves as a turning wheel and never falls prone. Opportunity attacks against it are made with disadvantage while it is hovering.

Magic Resistance. The ophanim has advantage on saving throws against spells and other magical effects.

Actions

Multiattack. The ophanim makes two Searing Spoke attacks and uses Gaze of the Seraph if it is available.

Searing Spoke. Melee Weapon Attack: +9 to hit, reach 10 ft., one target. Hit: 18 (3d8 + 5) radiant damage plus 7 (2d6) fire damage.

Gaze of the Seraph (Recharge 5-6). The ophanim's wheels blaze with revelatory light in a 30-foot cone. Each creature in that area must make a DC 16 Constitution saving throw, taking 33 (6d10) radiant damage on a failed save, or half as much on a success. A creature reduced to 0 hit points by this damage is utterly consumed in white fire.

Bonus Actions & Reactions

Roll Aside. When a creature ends its movement within 5 feet of the ophanim, the ophanim can use its reaction to move up to 20 feet without provoking opportunity attacks.

Legendary Actions (3/Round)

Spoke Lash. The ophanim makes one Searing Spoke attack.

Blinding Revolution (Costs 2 Actions). The ophanim spins, and one creature it can see within 30 feet must succeed on a DC 16 Constitution saving throw or be blinded until the end of its next turn.

Ascend (Costs 2 Actions). The ophanim flies up to its speed without provoking opportunity attacks and may use Searing Spoke against one creature it passes within reach.

A nested cage of fiery rings turns slowly above a bed of living flame, each spoke and rim crusted with open, lidless eyes that never blink and never look away.

Watchful Rim (signature). Allies within 10 feet of the Ophanim can't be surprised and have advantage on saving throws against being blinded.

Actions

Burning Spoke. Melee Weapon Attack: +7 to hit, reach 10 ft., one target. Hit: 14 (2d8 + 5) radiant damage.

Reactions

Shielding Spin. When an ally within 10 feet is hit by an attack, the Ophanim rolls between them, granting that ally resistance to the triggering damage.

A single smaller ring, broken from the host, that orbits a chosen champion and watches the angles they cannot.

Lead with Gaze of the Seraph on a clustered party, then split spoke attacks between casters and clerics. Use Roll Aside to slip grapples and stay airborne. Kill-priority is anyone trying to ground it with nets or hold effects.

Run it as a solo: open on the most clustered group with Gaze of the Seraph, then alternate Searing Spokes on the highest-damage PC. When bloodied, use Ascend each round to stay out of melee and force ranged exchanges where its truesight and disadvantage-gaze dominate.

Native CR 11 (7,200 XP) — a tier-3 solo built for 11th-15th level parties. Drop to the tier-2 (CR ~6) or tier-1 (CR ~2) line in the table to fit lower levels; the traits stay identical and only the numbers move.

Can Ophanim be a companion or ally?

Yes — use the Lesser Wheel, Kindled profile (AC 16, 117 HP). It orbits a chosen champion, shares its watchfulness so allies can't be surprised, and can roll between an ally and an incoming hit to grant resistance.
